Registration


How do I register?

We are always delighted to accept new patients who live in Westbury and the surrounding areas onto our practice list.

If you wish to register with us, please visit either of our surgeries. Speak to our reception staff and collect the relevant forms. Alternatively, you can click on the two links below, print the forms and then bring them to your surgery.

It would be a great help if you could bring with you your up-to-date medical card. If you cannot find it, simply see our reception staff who will give you a simple form to fill in. Please ensure you know the following information and are able to provide photo ID and a proof of address when you return your forms to the surgery.

  • Full name, including previous names.
  • Date and place of birth.
  • Last address.
  • New address and post code.
  • Last name and address of your previous Doctor.
  • NHS number if known. (This is not the same as your National Insurance number.)

If you are currently taking repeat medication please tell us this when you return your registration forms.


Does it matter where I live?

Yes. We can only accept patients who live within this geographical area as this allows us to make house calls when necessary. The Westbury Group Practice area covers Westbury, Dilton Marsh, Chapmanslade, Upton Scudamore, Heywood, Hawkeridge, North Bradley, Bratton, Edington, Erlestoke and Steeple Ashton.

On the map you will see two boundaries, If you live within the inner boundary you can register at the practice.

The outer boundary shows that patients who were registered with us but have moved between the inner boundary and outer boundary can remain registered with the practice if they wish to do so.

Patients who live or move to the area outside the outer boundary will have to register at a practice within their area. Please contact a member of the team if you have any queries.


Named Accountable GPs

All patients registered with Westbury Group Practice have a named accountable GP who is responsible for their overall care at the practice. If you wish to know who your named GP is, please contact the practice. If you have a preference as to which GP this is, the practice will make reasonable efforts to accommodate this request.
